HOW DO I OBTAIN A BIRTH CERTIFICATE WITHOUT COMING TO CITY HALL? Birth certificates can be requested from the City of Garden City by mail. A mail-in request form is available on this website. Your request can also be submitted in letter format. You must include the name on record, the date of birth, your relationship to the individual on record and an address where you want the record mailed. Your request must be notarized by a Notary Public with a stamp and a seal. Please include a check or money order for $15.00 for one copy and $5.00 for each additional copy made payable to the City of Garden City. Birth certificates can also be obtained on-line with the State of Michigan Vital Records at www.michigan.gov/mdch.
